Blank pads will not be provided. Labtests will provide pre-printed pathology pads with the referring practitioner’s own unique Dr Code to ensure you receive your patient’s results in a timely manner.

- Request a referrer code to be set up located by clicking this link. Please print the form, complete your details and email scanned form to lta.practitioners@labtests.co.nz
- Pathology pads can only be ordered once a referrer code has been created.
