Repeat laboratory test request forms

An audit of repeat request forms at Labtests has found that some patients have multiple forms with the same or similar tests on them, from different referrers. This can result in the patients having more blood drawn than necessary, and unnecessary tests are being performed. While there are valid reasons for repeat requests these are concerns that we wish to address.
